2013-3014 Kindergarten Enrollment

Thank you for your patience. Governor Sandoval has now signed all the pertinent legislation and we will proceed with Kindergarten Registration.

• We will accept applications on Thursday, June 27, 2013 from 11 a.m. to 1 p.m. at Oasis Academy.
• Immediately following we will conduct a lottery. Every complete application will be eligible for the lottery (no first come first serve). We will contact those drawn in the lottery via telephone.
• Only complete application will be accepted and eligible for the lottery! Please make sure you have the following items completely filled out:
  • Student Information Form
  • Student Special Needs
  • Student Health Needs
  • Authorization for Release of Records
  • Family Commitment
  • Home Language Survey
  • Copy of Birth Certificate
  • Copy of Immunization Record
  • Copy of Documentation of County of Residence (i.e. driver’s license or utility bill with your name on it)

We CANNOT make copies of immunizations, birth certificates, driver’s licenses or any other paper on June 27. You must have all copies made and attached to the application. You do not have to be a Churchill County Resident to attend Oasis Academy. All lottery exemptions allowable by law will be in affect.

Applications may also be dropped off at Oasis Academy during our normal business hours. Our business hours are 7:30 a.m. to 5:00 p.m. Monday-Thursday.

A complete application includes the following forms (available on the school website or at the school):

• Student Information Form
• Student Special Needs
• Student Health Needs
• Authorization for Release of Records
• Family Commitment
• Home Language Survey

The application must also include the following documents to be deemed complete:

• Copy of Birth Certificate
• Copy of Immunization Record
• Copy of drivers license, for address verification

Applications will be accepted at any time during school hours. When an opening occurs a lottery will be held. All eligible students will be notified of an upcoming lottery via email of phone. The lottery will occur in the presence of at least three persons including one member of the Board of Directors.

Applications can be submitted directly to the school or by mail. Please feel free to contact us if you have questions.

If at any time you choose to withdraw your student from Oasis Academy please do so in writing.
